Book at this locationPaired kidney exchange — also called kidney swap or kidney chain — gives patients a transplant even when their willing donor is blood-group or crossmatch incompatible. Dr. Anil Prasad Bhatt's team at Max Noida runs an active paired exchange registry that has facilitated multiple successful 2-way and 3-way swaps under India's Transplantation of Human Organs Act.
When your donor isn't compatible with you, the registry looks for another donor-recipient pair where the donors can be swapped — your donor gives to their recipient, their donor gives to you. Both transplants happen simultaneously to ensure fairness. The programme is fully legal under THOA when approved by the Authorization Committee.
Paired exchange is especially valuable when (a) the patient has high antibody levels (high PRA) making most donors incompatible, (b) blood group mismatch is severe (e.g., AB recipient with O donor), or (c) the patient has failed crossmatch with multiple donors. Evaluation takes 4–6 weeks per pair.
