(4) Creatinine above 1.2 mg/dL (men) or 1.0 mg/dL (women), or GFR below 60. (5) Urinating more than 2-3 times at night. (6) Family history of kidney disease, hypertension, or diabetes. (7) Diabetes with any protein in urine. (8) Persistent flank pain or visible blood in urine.
